GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:

The PASRR Habilitation Coordinator (HC) position is responsible for service planning, monitoring, assessing, identifying needs and desires relating to individuals with an intellectual and/or developmental disability who reside in a nursing facility or are at risk of being admitted into a nursing facility. The Habilitation Coordinator will also be responsible for providing Enhanced Community Coordination for individuals on their caseload. These responsibilities include the transition process of coordination with an individual who has requested to move into the community from a nursing facility. The Habilitation Coordinator will carry a caseload not to exceed thirty. The HC arranges and collaborates with service providers and nursing facilities to link individuals with outcome-focused specialized services. Habilitation Coordinators are responsible for scheduling and facilitating quarterly and annual meetings and completing all required paperwork. This position requires flexibility in scheduling and significant travel within the service area. The caseload will be primarily concentrated in the following counties: Atascosa, Frio, LaSalle, Dimmit, Zavala, and Maverick. Habilitation Coordinators are required to meet set productivity standards monthly. This position will be directly supervised by the Director of IDD PASRR and ECC programs.

MINIMUM JOB QUALIFICATIONS:
  • Bachelor’s degree from an accredited college or university with a major in social, behavioral health or human services field.
  • Must have two (2) years of habilitation/service coordination experience in providing service coordination to individuals with IDD.

PREFERRED JOB QUALIFICATIONS:
  • Bi-lingual in English and Spanish.
  • Reside in service area (Frio/LaSalle).

  • Knowledge of community resources
  • Must possess excellent verbal and written skills.
  • Ability to coordinate, plan and develop services as part of a team and individually.
  • Ability to develop productive relationships with individuals and their family members that focus on the individual’s specific goals and dreams.
  • Ability to advocate effectively for the best interest of the individuals served.
  • Ability to work cooperatively and effectively with other professionals, team members and other agencies.
  • Ability to work independently, organize work efficiently and communicate effectively.
  • Ability to meet stringent timeframes for submission of paperwork.
  • Ability to exercise tact, initiative, and good judgment in different situations.
  • Ability to complete documentation of contacts for billing purposes within 48 hours of the contact.
  • Must be able to complete all required contacts for assigned consumers monthly.
  • Ability to work under conditions requiring physical stamina and ability to work in a stressful environment.
