Charges for Services

The Sliding Scale charts below are used to determine your maximum monthly fee based on a number of factors, including the size of your household, your annual or monthly gross income, and any extraordinary expenses (such as medical expenses or property loss).  It is best to utilize this chart as a tool, as clinic staff will discuss this with you at the time of enrollment in services.
