Camino Real Community Services is a Trauma-Informed Care Agency and implements the principles of Trauma-Informed Care.

GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
Care Coordinators are relied upon to deliver the full range of services, to include integrated physical health and behavioral health services for individuals enrolled in Camino Real Community Services (CRCS).  Care Coordinators link individuals with primary care and provide transportation.  Care Coordinators are responsible for coordinating access to medical, dental, and other allied health professional care to advance the physical health care for individuals enrolled in CRCS.  Care Coordinators will coordinate individual physical health care within the entire CRCS team of behavioral health staff to include physician/other providers to facilitate resources and follow-up on unmet needs.  Care Coordinators will promote health eating/active lifestyles, and diabetes prevention/management, and prevent/control tobacco use, using evidenced-based practices.  CRCS priority populations served will include adults with severe, persistent mental illness (SPMI) and their families, to include those with dual substance use disorders (SUD), and children, and adolescents with qualifying mental/emotional conditions.  Care Coordinators collaborate with medical, dental, and allied health agencies, clinics, and private providers.  Care Coordinators coordinate, facilitate, tract, broker, and document referrals and appointments for medical services such as medical history and annual physical exams, medications/prescriptions, injections, laboratory tests, and any other medical services required or directed by physician’s orders.  Care Coordinators document work daily utilizing agency approved software.  Appropriate staff will provide clinical supervision.  Administratively, Care Coordinators report directly to the Integrated Health Care Program Director within the overall CRCS system.  Care Coordinators constantly exercise vigilance in maintaining confidential files and reports and contribute to CRCS state purposes.

MINIMUM JOB QUALIFICATIONS:
  • Bachelor’s degree from an accredited college or university with a major (or the equivalent to a major) in social, behavioral, or human services field with at least 24 hours of college credit in behavior/social science (psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education psychology, and early childhood intervention.
  • One year work experience as a case manager or case worker.  Each year (30 semester hours) of accredited college work in a related field may substitute for the required work experience on a year-for-year basis.


PREFERRED JOB QUALIFICATIONS:
  • Demonstrated ability to read, write and speak English and Spanish.
  • One (1) year of experience working with individuals with mental illness and/or dual diagnosis in a community setting.
  • Familiarity with psychiatric and mental health related terminology to include crisis response.
  • Work experience in a health care/clinic environment.
  • Resides in Wilson County.


KNOWLEDGE, SKILLS, AND ABILITIES:
  • Knowledge of individual needs/characteristics associated with severe and persistent mental illness.
  • Knowledge of community-based service delivery system and the role of case management.
  • Knowledge of community resources, how to access those services and use them effectively.
  • Knowledge of the principles, methods, techniques and practices of psychology, behavioral science, social work, chemical dependency, and the functioning of mental health hospitals and clinics.
  • Skill with needs assessments, treatment planning and responding to consumer’s changing needs.
  • Skill with contributing to a treatment team of physicians, nurses, counselors, and others.
  • Skill for independent, organized, and efficient work and effective verbal and written communication.
  • Skill for documenting work done as well as drafting reports and recommendation for administrative use.
  • Ability to work cooperatively and effectively with other professionals, individuals, and groups.
  • Ability to exercise tact, show initiative, and use good judgment in dealing with people even in crisis.
  • Ability to develop productive and outcome focused relationships with individuals and family members.
  • Ability to recognize and overcome interagency obstacles for accessing services for individuals.
  • Ability to identify problems/gaps in service.
  • Ability to advocate effectively for an individual in the target population.
